Please complete your registration by clicking the icon below and hand your Medical Eligibility Form in to the Nurses' Office.

**Please note that you will need your SIX digit TRENTON School ID # in order to register**
If you don't know your SIX digit School ID #, please contact your guidance counselor or the Athletic Office

If you attend an alternative high school - please use the "Trenton Central High School Athletic Registration" form

Physical Examination

Students registering for Athletics must have a current physical examination on file in the Nurses' office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, Medical Eligibility forms must be turned into the Nurse prior to the first day of practice/tryouts. 

Physicals are valid for 12 months from the date of the last exam.
